My phone get stolen and I will report aclaim beside Asurion. But I assume Ive file more than 2 claims contained by 12 months

My mother left my phone at K-mart the first time, and it be stolen.
The second time it broke really bad...but after again I'm not sure if I filed a claim or not because I remember taking it to the VZW store and they give me a new phone for free...but I kept my productive battery...
My quiz is if there is a excise for more than 2 claims in 12 months and how much it costs...
