Answer me put a bet on...what state did this accident take place in....
Different insurance rules apply from state-to-state as here is no unifying federal insurance guidelines.
There are also different programs within different states.
There may be something on his or other family member auto policies that may help.
---edit info---CALIFORNIA
This may not be set to too much to you at this point in your vivacity but I'll keep it to some extent simple...
California is not a "No-Fault" or "PIP" state so the automobile coverage will be limited as follows:
Automobile MedPay policy:
He could record a Medical Payments claim under his own auto policy.
He could database a Medical Payments claim under the vehicle owner's policy if it be someone else's car
He could report a Medical Payments claim under the policy of a household member beside whom he resides.
These auto MedPay policies usually have confines of $1,000-$10,000 and will help somewhat.
Second, if the disaster was not his glitch...
(1) He could file a liability claim against the at guiltiness party's insurance carrier.
(2) If the twist of fate was cause by a hit and run driver that cannot be identified he coould possibly file an uninsured motorist claim against his own auto policy.
If this friend be working at time...he could file a Workers Compensation claim on his enployer's WC holder.
Each CAlifornia County has an indigent aid program on top of Medicare and Medi-Cal programs offered at federal and state level.
This male friend's best choice of coverage for this is "Medi-Cal" if here is no automobile or Workers Compensation policies applicable to accident.
Below are links for (1) AM Best digest of state insurance law... Very Dry reading.
(2) California Medcare program
(3) California Medi-Cal program
Hope this shows you how important it is to be fully insured surrounded by case this happen later to you.

One word "screwed" but, most hospitals hold a charitable assistance program i know the hospital i work at does. As long as his income falls below the lowest level they will pick up the unbroken tab :)
Have a nice day :)
you should ask a financial expert over at http://www.MechanicsofMoney.com The answer from Fryguy93 is the most accurate one that you have received so far. I own a couple of additional suggestions.
1) Visit the social services department of the hospital where on earth the surgery would be done. Explain the situation. They will be in a biddable position to refer you to programs that may help. Watch out if you are asked to sign any documents that obligate you to retribution for the surgery.
2)Find out if the car catastrophe was work related. If it be work related then it may be covered by the employer workers compensation policy.
3)You can always review your situation beside an attorney who can help you detemine if anyone else is responsible for the chance. That kind of warning may lead to a financial reclamation. Many lawyers will not charge a payment for an initial conference with a client.
Fryguy93 is right around different laws within different states. Depending on where the coincidence happened in that may be a program or insurance that will help you.
Good Luck
