Usually, the Medicaid # is not the same as the Medicare or SSN. You should contact the department which handle Medicaid, usually called "Health and Senior Services" department. Many big hospitals also enjoy a Medicaid office/social worker on staff. You can walk into any of these hospitals and ask for a replacement Medicaid card.
